There is a growing appreciation among health and social care providers, especially those working in community -based programs with women or young people, that see social support as critical to having a healthy pregnancy. We agree.
Here’s how we roll out support in our region.
LOCAL PEOPLE, LOCAL SUPPORT: PROMOTES LIFELONG SUCCESS
PCAP | Parent Child Assistance Program
PCAP is a three-year mentorship program that supports vulnerable pregnant women and mothers up to six months after they’ve given birth. We have several PCAP providers within our region that provide local and culturally relevant support. Support can include helping you access housing and income support, child custody, health education and prenatal care. For others, it may be touring the hospital to prepare for delivery, connecting you with cultural and spiritual supports, or simply being there to talk.
COORDINATED INTAKE: INTEGRATED & FOCUSED HELP
Our Resource Coordinator’s aim is to identify assessments & services within your community that align with (and best support) your individual needs. Support can include family, life and community living skills, assistance in developing strategies to be successful in the workplace or learning environment, or approaches to reduce secondary disabilities such as trouble with the law, addictions or mental health problems. For others, it may be support to fill out forms to access disability services or some advice on safe and healthy places for social connection, like The Circle Series:: Circle of Support & Arts.
Since every woman we serve is different, we’ll have a conversation to determine your needs (what we call centralized intake). Then, we’ll connect you to the help you need from our collective of resource and service providers in the region.
